consultations carry the same professional obligations as in-person visits.
3. Service Availability and Scheduling
While we strive to offer same-day appointments, service availability depends on specialist
schedules and patient volume. Scheduled appointments are subject to change. We reserve the right
to reschedule appointments due to emergencies or operational requirements with adequate notice.
4. Payment and Insurance Terms
Patient responsibility for payment includes copays, deductibles, and non-covered services as
determined by your insurance plan. We accept major credit cards, HSA/FSA cards, and offer
payment plans for qualifying balances. Unpaid balances may be subject to collection proceedings.
5. Cancellation and No-Show Policy
Appointments must be cancelled or rescheduled at least 24 hours in advance. Late cancellations
and no-shows may incur a fee of up to $75 for standard appointments and higher for specialized
procedures. Repeated no-shows may result in care limitations.

9. Patient Conduct and Responsibilities

staff with respect. Abusive, threatening, or disruptive behavior toward any Pure Health Studio member
will result in immediate termination of the patient-provider relationship.
10. Limitation of Liability
Pure Health Studio's liability is limited to the extent permitted by applicable law. We are not liable

indirect, incidental, or consequential damages arising from the use of our services. Our total
liability for any claim shall not exceed the amount you paid for the specific service in

may proceed to binding arbitration in USA.
